Neck Pain, Headaches or Stiffness After a Car Accident?
Whiplash can occur when a collision rapidly moves the head and neck. Symptoms can affect more than the neck and may interfere with driving, work, sleep, exercise and everyday activities. Our Surrey and New Westminster physiotherapists assess movement, strength, function and symptom behaviour, then develop a rehabilitation plan around your individual presentation and recovery goals.
Whiplash Physiotherapy • Neck Pain • Headache Rehabilitation • Back & Shoulder Symptoms • Dizziness Assessment
What Does Whiplash Feel Like?
Symptoms vary from person to person and can change during recovery. Common complaints after a collision include:
- Neck pain, stiffness or reduced movement
- Pain when turning the head, including while driving
- Headaches associated with neck symptoms
- Upper-back or shoulder discomfort
- Lower-back pain after a collision
- Muscle tightness, tenderness or fatigue
- Pain with desk work or prolonged sitting
- Difficulty sleeping or finding a comfortable position
- Reduced tolerance for work, exercise or daily activities
- Dizziness or balance symptoms requiring appropriate assessment
What Is Whiplash?
Whiplash is an acceleration-deceleration injury of the neck that may affect muscles, joints and other soft tissues. A physiotherapy assessment helps identify current movement and functional limitations, screen for findings that may require medical attention, and determine an appropriate rehabilitation approach.
Rather than assuming every post-collision symptom is caused by whiplash, assessment considers the location and behaviour of symptoms, movement, strength, neurological findings when relevant, and the activities that have become difficult.
Can Whiplash Symptoms Appear Later?
Some people notice pain or stiffness immediately after a collision, while others become more aware of symptoms later. If new or worsening symptoms develop after an accident, they should be assessed appropriately. Severe, unusual or rapidly worsening symptoms require medical evaluation.
Whiplash Recovery: RELIEVE → RESTORE → RECOVER
1. RELIEVE
Settle Symptoms & Restore Comfortable Movement
Early rehabilitation focuses on understanding your symptoms, maintaining appropriate movement and helping you manage activities that aggravate the neck, upper back or shoulders. Treatment is individualized rather than based on a fixed whiplash protocol.
2. RESTORE
Improve Mobility, Strength & Control
As tolerated, rehabilitation progresses toward neck and upper-back mobility, postural control, strengthening and functional exercise. Manual therapy, IMS/dry needling, BioFlex cold laser or Class IV laser may be used when clinically appropriate.
3. RECOVER
Return to Driving, Work & Daily Life
The later phase emphasizes confidence with normal movement, progressive strengthening and tolerance for driving, work, exercise and daily activities. The goal is to restore function rather than rely indefinitely on passive treatment.
Whiplash, Neck Pain & Headaches
Headaches are a common complaint after a motor vehicle collision, but they can have several causes. When symptoms are associated with neck movement or musculoskeletal dysfunction, rehabilitation may include mobility work, strengthening, manual therapy, education and gradual activity progression. Symptoms that suggest another cause may require additional assessment.
Shoulder, Upper-Back or Arm Symptoms
A collision may also produce upper-back or shoulder pain. Some people report pain, tingling or numbness into an arm. These symptoms should be assessed rather than automatically attributed to whiplash. Your physiotherapist can screen movement, strength and neurological findings and recommend medical assessment when appropriate.
Back Pain After a Car Accident
Whiplash describes an acceleration-deceleration injury involving the neck, but a collision can also produce lower-back symptoms. If back pain, buttock pain or leg symptoms are a major concern, assessment can help determine the appropriate rehabilitation approach and whether further medical evaluation is indicated.
Dizziness, Balance Problems or Concussion Symptoms
Some people experience dizziness, balance problems, headache, visual sensitivity or motion sensitivity after a crash. These symptoms may require vestibular or concussion-focused assessment rather than routine neck treatment alone.
- Surrey: vestibular and concussion rehabilitation is available for appropriate post-collision presentations.
- New Westminster: vestibular and vertigo assessment and rehabilitation is available. Concussion rehabilitation is provided through the Surrey clinic.
What May Whiplash Physiotherapy Include?
- Education and guidance for return to normal activity
- Neck, upper-back and shoulder mobility exercises
- Progressive strengthening and postural control
- Manual therapy when clinically appropriate
- IMS/dry needling for selected musculoskeletal presentations
- BioFlex cold laser or Class IV laser when appropriate
- Balance or vestibular rehabilitation when indicated
- Home exercise and self-management strategies
- Progressive return to driving, work, exercise and recreation
How Do We Know Whiplash Is Improving?
Recovery is not judged only by a pain score. Your physiotherapist may monitor changes in:
- Neck range of motion
- Strength and movement control
- Headache frequency or symptom behaviour
- Tolerance for sitting and desk work
- Ability to turn the head while driving
- Sleep and daily activity tolerance
- Work and exercise tolerance
- Balance or dizziness when relevant
Progress is reviewed over time so rehabilitation can be adjusted to your response and functional goals.
Should I Exercise With Whiplash?
Complete prolonged rest is generally not the objective of rehabilitation. Appropriate movement and progressive exercise may form part of recovery, but the type, intensity and timing should match your symptoms, assessment findings and any medical restrictions.
When Can I Return to Driving, Work or the Gym?
There is no single timeline that applies to everyone. Return to activity depends on symptoms, movement, strength, functional demands and recovery progress. Rehabilitation can be progressed toward the specific activities that matter to you, including driving, work duties, exercise and sport.
